minimum for Lead Information:
First and Last Name
Email Address
Address, City, State and Zip Code
Phone and Cell Phone
What Custom Form Fields gives each Radius User is the ability to Add, Edit or Remove Custom Form Fields for Leads and Clients. Go above and beyond basic contact info such as Height, Weight, DOB, Number of Bedrooms, Annual Revenue and more. We know that each Agent has their own set of requirements for Data they want to collect for a Lead which is why Custom Form Fields is a great feature.
Custom Fields gives you the ability to define Lead and Client Coverage Type Forms.
Height, Weight, Number of Bedrooms, Employees, Gated Community or whatever you might need, create at will.
Modify Health, Home, Renters, Condo, Annuities, Auto or Life Coverage Type Forms
Field Name - Name of the Data Field to be created within your database (Height, Weight, DOB)
Description - Descriptive Name of the Field Name being Collected (Height, Weight, DOB)
Sort Order - Numerical value applied for Custom Field position (1,2,3,4..)
Choose a Coverage Type - Which Coverage Type Form to apply new field to
